This looks like a typical testing ground design mistake. You wouldn't believe the US videos that came out from cold war archives over the years when the US was trying everything and anything to get the edge and thus letting their engineers try all sorts of crazy stuff.

I'm suspecting this may be old, possibly dating back to the 80s. Still typical of Russia back then.   They are known for not having proper safety measures in their testing. In the 80s it would have been for cruise missiles.
