My 91 yo father w/a broken back was kicked out of rehab bc he had medicare advantage. When I went to apply for medicaid, the nursing home rep told me to pay them instead of the $7K I racked up on his Amex to pay for home care bc insurance threw him out.
I've seen 1st hand who is committing fraud.
