Almost every Apollo mission had issues. Apollo 13's second stage blew up mid-flight and nearly killed the crew. SaturnV was prone to pogo oscillations and it's a miracle no F-1 engine blew up during a mission. SaturnV being so unstable was one of the main reasons for its retirement.
